SubhanAllah..

Ramadhan kareem, sebenarnya bukanlah hobi untuk menulis.Hanya akan menulis pabila ada sesuatu yang ingin disampaikan. Tulisan akan menggambarkan 'mood n affect'. Emosi sekarang = tidak stabil.Tidak perlu tanya kenapa, tuan badan sendiri pun tidak mengetahui.Hanya Allah sahaja yang mengetahui.

Satu ilmu i'Allah akan dikongsi bersama, Ini pun dapat daripada satu majlis tazkirah ringkas selepas 4 rakaat tarawiH kelmarin.Berkenaan lafaz agung nama Allah Subhanahuwata3ala (SWT).

Keagungan Allah SWT disematkan pada kalimah Allah atau  الله. Antara keajaiban perkaatan yang membawa maksud tuhan sekalian alam ini adalah jika kita menghilangkan satu huruf yang ada padanya maka tidak akan hilang makna Allah atau tuhan sekelian alam.  

Pertama, pabila kita menghilangkan huruf Alif yang pertama, akan menjadi لله bacaannya Lillah.

(4:170)

Kedua, Jika kita menghilangkan huruf Lam tidak kiralah yang pertama atau kedua, kalimah suci ini akan menjadi إله bacaannya ilah.

(2:163)

Yang terakhirnya, pabila kita menghilangkan huruf Ha, Ia akan menjadi إلا bacaannya illa.Akan tetapi disambung dengan sifat-sifat atau sesuatu yang hanya layak bagi Allah.

SubhanaAllah, benarlah kalamMu wahai tuhan sekelian alam yang diturunkan di dalam Al-Furqan melalui kekasihmu juga rasulMu Muhammad Sallahhi 3alaihi wasallam.

jika terdapat manfa'at maka sebarkan,

2nd Story

Normal elbow x-ray

For those who are unfamiliar with medical terms, there are medical term in this post which I'll simply explain just for the benefit of understanding.The story will not be understood without knowing the terms.

First the physical therapy, it is a therapy of muscles and joints done by the physical therapist to restore weak, or atrofied muscles to their healthy state by using massaging, heat and by electical stimuli methods.

The second term is Myositis ossificans, is a pathological calcification of healty muscle and hardened it limiting muscle or joint movement.Also in worse case scenario the muscle affected  will become bone-like.

Fuhh, many to explain right? i'll continue with the story...,the title is..,

Boy without teeth!  

A worried mother and his son came to Dr Ali el-deeb clinic, a specialist in rheumatology and an assistant professor at the Tanta University Hospital (TUH).

The boy is wearing a splint on his right elbow, he recently suffers from a trauma that cause  a fracture on his upper one third radial bone. 

The fracture is Alhamdulillah cured, but the mother's complaint is that her son can not move his right arm, due to stiffness on his elbow joint.

It's normal for any joint to has this kind of affect after being cast by plaster of paris (pop), due to the stagnant synovial fluid and bla..bla..bla..(no time to explain..maybe another time)

After assessment Dr Ali noticed that all of the boy's front teeth were missing.So he ask the mother is it due to the accident.Shockingly the loss of teeth had nothing to do with the accident.

Want to know???

It's horrifying, even to Dr Ali! the child was undergoes a physical therapy for his stiff joint, and the most important therapy is the joint stretching which is flexion and extension of the affected elbow joint.

The therapists was doing it without doing a routine x-ray, a simple investigation.They stretch the joint without knowing that the boy has myositis ossificans of brachialis muscle!!.

His muscle had completely turn into bone, can you imagine when someone try to break your bone!.Scream! that is the word, but in this case the boy kept on screaming but the therapists said it is normal (they don't know about the calcified muscle).

The boy kept on screaming and bite his teeth together to ease the pain, sadly they kept on stretching his arm till the calcified muscle broke!.The boy can not stand the pain any longer and bite his front teeth.All the teeth broke due to the straining and the agonizing pain.

There is blood all over his face and after seeing that they stop the stretch.

The mother and the therapist were confused, they do not know what had happened.

Thus the mother became angry and they leaved the therapist out of the blue.A day after,  she visited Dr Ali's clinic and Dr Ali explains the condition of the poor child after confirming it with a simple elbow joint x-ray.

His calcified brachialis muscle was broken, so the choice of treatment is surgery and his arm will not able to move like before due to iatrogenic or his previous doctor silly, stupid yet fatal mistake.Don't forget that his front teeth are all gone too!.

1st medical dilemma

Tinea capitis grey patch vs localized Alopecia areata are my first medical dilemma.The medical crossroad to consider which is which.It needs high level of understanding of both diseases.As both look the same, a patch of hairless and circumscribe lesion. 

It confused me a little, because the initial lesion is almost the same and the exclamation site is very hard and almost impossible to find.Also the diagnostic scales of the tinea capitis often get wash away by shampoo (mothers always clean her offspring before seeing a dermatologist).

Without the characteristic scales, patch of tinea capitis will look like patch of alopecia areata!.The solution is, tell the mother to come back or re-inspection after 3 days and tell her also do not wash her offspring hair.

Then the sign and scales of tinea capitis will return the diagnosis is surely tinea capitis and if it does not return, it is the same like before then the diagnosis will be alopecia areata.

Albeit that this is only a dermatological case, a misdiagnosis will be an eternal error because the treatment of both disease are totally contraindicated to each other.As for tinea is antifungal and for alopecia is corticosteroid.

Corticosteroid treatment in tinea capitis will flare the infection causing diffuse tinea capitis.My lecture advice is "Do not jump into a diagnosis like you jump from a bus here in egypt, but slowly arrive at the station and take all the steps necessary then after that you'll arrive at the diagnosis in no time at all".

In Neurology round, one of the most typical and classical case is hemiplegia. For those who are still not accustomed to medical term it is called one-sided paralysis according to the part of brain affected.

"What is common is common" a sentence which my lecturer repeat all and over, because in medical field, we as a student or a HO or a MO or the upper level doctors must exclude the typical causes and then the uncommon or the atypical causes are considered after the commons were subsided.

Vascular is the common cause of hemiplegia.The 'tiga serangkai' illnesses which are Diabetes Mellitus type II-Hypertension-Arteriosclerotic Heart Disease.Enough of the medical mumbo-jumbo, want more info just click on the links.

In hemiplegic patient the pyramidal tract affection will cause weakness in the extensors of the upper limbs and flexors of the lower limbs.So the patient attitude will be like this ( the patient suffers left hemiplegia ) .

left lower limb in extensor attitude 

These position is a blessing from Allah because the flexion of the upper hand helps him to touch his mouth, eats and wash his face.Furthermore the extension of the lower limb allows him to walk ( Subhanallah ).

Allah's blessing is everywhere, open our eyes, mind and heart.We can sense it if we try!.A lesson to learn is concluded in the ayah below, from surah Al-An'am verse 12.

"He ( Allah ) has decreed for himself mercy"

Keluar waktu malam la best musim2 panas ni, sejuk sket kalo klua pagi adoi mmg xtahan panas, terik, dan berlecaklah tubuh badan.Ikhwah je yang boleh klua malam kat mesir ni, akhawat tidak boleh kecuali hal-hal kecemasan dan penting sahaja.Tapi, ade je yg klua tanpa mushrif, nt bl da jadi pape nt tanggung sendiri k! (bukan nk doakan ke ape ke, pikir sendirila yea da besa).

Umah ni lengkap ssgt daripd thn 1 hingga thn 5 ade, 2 orang tahun 5, sorg thn 4, sorg thn 3, 2 org thn dua, dan sorang lg payess tahun 1 (duk uma atas tp kami da anggap mcm ahli beyt sbb bl bunyi loceng mesti muka die, cume skang jarang dtg sbb nk xm, tp tgu la world cup esok cmne---).

Me ( ngarut gler, da la dalam muhadarah ramai org maluuunye kalo salah ) 

"Erm..,erm..,to exclude Malignancy ya dukturah"

"Aiwah asaH keda" said her. ( tepat skali )

Lesson to learn?

1.Always study because luck doesn't work multiple times.

2.Always try to improve your Ammiah ( increase in years doesn't mean increase fluency-la syafiq! )

3.Always hide yourself from being ask by Dr ( just for myself )

4.Do not become the doctor in the stories below ( very important )

Alhamdulillah selesai sudah satu 'major' subjek saya pada tahun ini.Kalimah suci alhamdulillah yang saya ucapkan ini bukanlah petanda kepada kemenangan atau pengakhiran akan tetapi ia adalah satu petanda bahawa perjuangan 3ilmu yang tidak kunjung putus ini sudah sampai kepada fasa akhir bagi sesi 08/09 tahun 4 saya di sini.Apabila dikenangkan kembali sudah 4 tahun saya berada di sini,mendalami ilmu Allah yang diberikan secebis cuma kepada hambanNYA yang amat lemah ini.

Astaghfirullahal3azim pabila saya mengenangkan pemergian sahabat kita semua yang baru sahaja menemui Allah (moga dibawah naunganNYA) , saya disini masih lagi mendapat nikmat ukhuwwah islamiah yang masih utuh dihati mahasiswa Tanta perubatan juga julusan agama.Malunya saya kerana tidak mensyukuri nikmat ukhuwwah ini yang mana mengiringi nikmat iman dan islam..Allahuakbar,saya amat berharap akan almarhum akan sentiasa di ingati di dalam doa harian kita,"kenanglah daku dalam doamu" itulah yang akan saya ucapkan jika sayalah yang menemui ajal.

Pada suatu hari ada sekumpulan katak-katak kecil,...

… yang berlumba-lumba

Tujuannya adalah mencapai puncak sebuah menara yang sangat tinggi.

Penonton berkumpul bersama mengelilingi menara untuk menyaksikan perlumbaan dan memberikan semangat kepada para peserta...

Perlumbaan pun dimulakan...

Secara jujur:

Tak satupun penonton benar2 percaya bahawa katak2 kecil akan boleh berjaya mencapai puncak menara.

Terdengar ada yang berkata:

"Oh, jalannya terlalu susahhhhh!!

Mereka TIDAK AKAN BOLEH sampai ke puncak."

atau:

"Tidak ada kesempatan untuk berjaya...Menaranya terlalu tinggi...!!

Katak2 kecil mulai berjatuhan. Satu persatu...

... Kecuali mereka yang tetap bersemangat menaiki menara perlahan- lahan semakin tinggi...dan semakin tinggi..

Penonton terus bersorak

"Terlalu susah!!! Tak seorangpun yang akan berjaya!!!"

Lebih banyak lagi katak kecil penat dan menyerah...

...Tapi ada SATU yang tetap melangkah hingga semakin tinggi dan tinggi...

Dia tak mahu menyerah kalah!

Akhirnya yang lain telah menyerah untuk menaiki menara. Kecuali satu katak kecil yang begitu berusaha keras dan menjadi satu-satunya yang berjaya sampai ke puncak!

SEMUA katak kecil yang lain ingin tahu bagaimana katak ini boleh melakukannya?

Seorang peserta bertanya bagaimana cara katak yang berjaya itu mempunyai kekuatan untuk mencapai tujuan?

Ternyata...

Katak yang menjadi pemenang itu TULI!!!!

Nasihat bijak dari cerita ini adalah:

Jangan sesekali mendengar kata orang lain yang mempunyai kecenderungan negatif ataupun pesimis...

…karena mereka akan mengambil sebahagian besar mimpimu dan menjauhkannya darimu.

Selalulah fikirkan kata2 bertuah yang ada.

Karena segala sesuatu yang kau dengar dan kau baca boleh mempengaruhi perilakumu!

Kerana itu:

Selalulah tetap....

Dan yang terpenting:

Bersikap TULI jika ada orang mengatakan bahawa KAMU tidak boleh mencapai cita-citamu!

Exaggerated night itch, intense scratch marks, pleomorphic lesion, pubic region affection in males, mamary region affection in females, and more than one member of family involved are signs and symptoms of a very well-known ectoparasite infection scabies.    

Every one out of five patients in my Dermatology clinical round are suffering the affect of harbouring these malicious being.They are very contagious, and can infect you either direct ( direct contact ) or indirectly ( share the same clothes, towels, etc ).

Incognito scabies is a modified type of scabies infection.It is resulted from abusing topical corticosteroid or commonly used in Egypt which is Betaderm, it can alleviates the itch, minimizes the inflammation and suppresses the allergic reaction but the side effects are more graver than it benefits.

Incognito scabies will clouding dermatologist diagnosis of the lesion, because of the atypical presentation of initial lesions and it decrease the immune reactions ( which is good ) to protect our body.Losing the immune reactions are the main contraindication of Betaderm.

Do not ever use it without consultation from a dermatologist as it can also leads to various and terrifying effects.The common misuse in Egypt is when rich in melanocyte woman who wants fair skin, they misuses Betaderm for its blanching properties, so she will look whiter than before.But beware of the consequences that are cortocosteroid-acne that is very difficult to treat.Think twice before usage!.

Do not prescribe your friends or buy over the counter Betaderm without doctors prescription.Some of the pharmacy will give you Betaderm for every skin related infection due to its action mentioned above.It will only treat the symptoms not the real infections,and may also even exaggerated it! So beware of this warning.Go to the 'real doctors' and pay to get 'real' medical services.

Biadap!..sungguh biadap mereka.Berani menghina junjungan kita Nabi Muhammad SAW secara terbuka, anggapan mereka perkara tersebut adalah jenaka.Tetapi bagi kita selaku muslim yang mengaku beriman kepada kemuliaan Allah SWT dan kemuliaan Nabi kita Muhammad SAW, perkara ini sewajibnya di tentang habis-habisan.

Ikutlah cara apa yang di rasakan perlu, petition, boikot dan 'freeze' akaun facebook masing-masing dan sebarang aktiviti bagi menunjukkan rasa tidak-puas-hati, protes kepada yang si biadap yang bertanggungjawab memulakan kempen tidak berakhlak ini. Isu ini adalah isu ummah!, wajib kita mempertahan kan nabi yang kita mengaku cintai,ia juga adalah isu maruah! jangan benarkan sewenang-wenangnya menyamakan Rasulina dengan karikatur yang berniat mengeji!.

Observing is not an action!..,Defend our beloved prophet!..,increase our faith level from 'أضعف الإيمان' !

Hadith nabi Muhammad SAW,

يقول صلي الله عليه وسلم " من رأي منكم منكرا فليغيره بيده فإن 

لم يستطع فبلسانه فإن لم يستطع فبقلبه ، وذلك أضعف الإيمان 

(متفق عليه )

Hanya ingin mengutarakan jawapan bagi satu komen yang saya fikir perlu diperjelaskan dengan lebih terperinci.Mahupun penjelasan yang terperinci tidaklah begitu sinonim dengan saya tetapi melalui ilmu yang sedikit di dada ini, rasa ingin berkongsi kefahaman dan mempunyai siratan harapan  untuk membuka belenggu fikiran kita yang telah diserang oleh pemikiran barat selama ini.

Apakah contoh pemikiran barat? dan apakah hasil yang ingin mereka capai?

Seluruh 'ism-ism' adalah cabang daripada 'secularism' yang bermaksud pekeliling atau sesuatu yang mengelilingi, contoh MRR1 ( middle ring road ) atau jalan Tun Razak di Kuala Lumpur, jalan ini satu ketika dahulu dinamakan Secular road yang membawa maksud jalan yang mengelilingi.Lebih tepat lagi ia adalah jalan yang mengelilingi juga memisahkan antara Kuala Lumpur dan kawasan negeri selangor yang berhampiran.

Secularism moden yang dibawa oleh firaun moden Kamal Atarturk di Turki membawa agenda untuk memisahkan agama dan kehidupan duniawi.Ia amat bertentangan skali dengan ciri islam yang utama iaitu ad-deen membawa erti 'the way of life' islam adalah cara hidup.Pemikiran ini perlu ditolak sama sekali jika kita menggelar diri kita seorang muslim.

Patriotism ataupun bagi saya adalah 'selfish-ism'.Ini juga adalah pecahan pemikiran barat yang dalam strategi peperangan namanya 'divide and conquer'.Anggaplah saya seperti perancang barat atau firaun moden yang ingin menakluki satu kawasan.Perhatikan monolog berikut.

Jelas bukan patriotism adalah 'selfish-ism' yang bersaiz besar sebesar satu negara!.Bagaimana sifat pentingkan diri yang amat negatif dianggap sesuatu yang positif dan diterima oleh masyarakat di seluruh dunia.Haaa..,itulah tandanya minda kita telah diserang dengan hebatnya oleh mereka-mereka yang mempunyai tujuan jahat.

Mana ukhuwwah kita? mana kasih sayang kita? yang diajar oleh Rasulullah (saw) dalam sunnah baginda.

"cuma ingin bertanya lebih dalam, apakah yang "sebenar"nya dimaksudkan dengan "unity" dalam Islam mengikut hadith-hadith Rasulullah?PCT perlu mandiri, tidak boleh dipengaruhi oleh mana-mana pihak, baru semua pelajar perubatan tanta akan bersama. Tapi, kalau PCT nampak berat ke arah mana-mana persatuan di mesir ini, maka para pelajar tidak lagi berminat dengan PCT."

Jawapan saya:-

saya bukanlah orang yang layak untuk membicarakan akan hadis-hadis nabi muhammad..

tidak layak,bahkan tidak mampu..begitulah lemahnya saya ini..

tetapi pandaagan saya adalah berdasarkan pandangan ulama' yang mana adalah pewaris nabi Muhammad SAW. Fatwa sheikh Qardawi, dalam membicarakan hal ta3adud jemaah atau berbilang-bilang persatuan.saya hanya menyandarkan hujah saya kepada beliau.


beliau mengatakan bahawa ta3adud jemaah adalah HARAM apabila 'tudhod'(berlawan antara satu sama lain) dan hanya dibolehkan pabila 'tanawwu3'(bercabang cth dlm satu persatuan ade jage kebajikan,akademik n sbg) atau 'ta3awun' (yang tolong menolong)

saya xkata kepada mana-mana persatuan secara spesifik..,akan tetapi itulah pegangan saya.
kalimah ini daripada seorang ustaz kenalan saya dan saya thiqah padanya.
Dalam satu kaedah fiqh
'tahsil hasil batil'
menghasilkan benda yang baru yang sudah ada sebelum ini adalah salah.


PCT tidak berkompromi dengan mana-mana persatuan,hanya di muhafazah kami perlukan DPMT hanya untuk urusan keselamatan,perumahan dan ukhuwwah kami selaku pelajar yang belajar bersama-sama dan membesar bersama-sama di bumi Allah tanta ini.ini pendapat saya,yang baik dr Allah...Wallahua'lam
